(RN) Registered Nurse - Home Health | Hillsboro, Oregon

An exciting opportunity awaits experienced Registered Nurses passionate about home health care. In this dynamic field-based contract, youll provide essential support to patients in their homes, making a meaningful difference every day.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

• Full-time, 13-week contract

Role Overview:

• Daily travel to patient homes throughout your assigned territory (mileage reimbursement provided)

• Caseload averages 35 patient visits per day, with a weekly goal of 30 points

• Utilize the Home Care Home Base (HCHB) electronic documentation system and OASIS assessment tool

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans

• Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management

• Monitor patient progress, report condition changes, and coordinate with physicians and interdisciplinary team members

• Educate patients and their families on treatment plans, disease management, and preventive health strategies

• Maintain detailed and accurate patient records in HCHB

• Collaborate with therapists, social workers, and aides for comprehensive care delivery

• Participate in team meetings, case conferences, and ongoing education

• Ensure compliance with all regulatory and agency care standards

Preferred Qualifications:

• Current, active RN license in the state (in good standing)

• Minimum of 1 year of recent home health nursing experience (field experience preferred)

• Proficiency with Home Care Home Base (HCHB) and OASIS

• Dependable vehicle, valid drivers license, and proof of auto insurance
