In-Home NP/PA Clinician — Personal Health at Home

Signify Health, Inc. is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform In-Home Health Evaluations, engaging with patients in the comfort of their own homes.

You will build trusted relationships, perform medication and medical history reviews, conduct physical evaluations, and support diagnostic services as ordered by health plans, traveling locally within licensed states and conducting occasional virtual visits when needed.
