Hybrid Senior Social Worker, Medi-Cal & Community Resources

Blue Shield of California is seeking a Senior Social Worker to outreach to Promise Health Plan members, perform psychosocial assessments and provide intensive interventions. The role links members to community resources to reduce social barriers and help them manage medical care while staying stable in the community.

Candidates should have a MSW and LCSW licensure, with CCM preferred and at least 3 years’ experience working with Medi-Cal populations.
