Care Coordination Specialist

Northern Montana Hospital is seeking a Health Care Coordinator to assist in managing patients, especially those with chronic disease. You will perform population health, care management, and patient-self management support, and help link families to community resources.

The role emphasizes collaboration, advocacy, and improving care continuity across NMH’s rural Hi-Line service area in Montana. Health coaching and BLS certification are valued assets.
