We are seeking dedicated Social Workers to provide essential care management services at Providence Alaska Medical Center.

Available Schedule:
• Full time, Day shift, 40 hours per week.

$10,000 Hiring Bonus available for eligible external hires that meet required qualifications.

Relocation Assistance is offered for eligible hires.

As an Inpatient Social Worker, you will deliver comprehensive, patient-centered care management services to at-risk patients in an acute care setting. Your responsibilities will include care coordination, discharge planning, accurate documentation, compliance with regulations, and patient advocacy. The primary goal is to empower vulnerable patients by addressing social, financial, cognitive, behavioral, or legal obstacles to accessing care.

Required Qualifications:
• Bachelor's Degree in Social Work from an accredited program.
• Alaska Baccalaureate Social Worker License upon hire.
• One year of social work experience in a healthcare setting, specifically in a medical facility or long-term care environment.

Preferred Qualifications:
• National Certification in a relevant specialty.
• One year of experience in an acute care setting or successful completion of a case management training program.
• Basic understanding of medical terminology and previous case management experience.
